When Navigating to the Organization Profile under advanced settings, you will notice there are 4 different names.

Program Defined Facility Name: This is the name that has been determined by the PA Department of Health. This is the name they use in reporting. PLEASE DO NOT CHANGE. PADOH monitors this name.

Organization Name: This is the name that all the Facilities are housed under when the site is built. This is usually the most common name that is used when referring to the facilities. This cannot be changed.

Facility Name: This is the name of each facility that is used and is present in LEAF. This is the name that appears in the upper right corner of platform. This is also the name that other facilities will look for when sending transfers. This can be changed in LEAF &amp; Platform, however it is not recommended as it may cause confusion for facilities sending you product.

Legal Business Entity Name: This is the name that is on file at the Department of State as a Fictitious Name (other states may call this a DBA or Trade Name). If your Permit is under a Business Entity Name that is not included elsewhere, it is best practice to include this here as well (Business Name – Fictitious Name). This can be changed but the name filing should be completed and showing on the Dept. of State website prior).
