Custom Test Result Fields

Applicable to all jurisdictions except Pennsylvania and Utah.

Added five custom fields to the bottom of the Add Test Results page: Custom 1 through Custom 5. You can use these fields for any custom test result you want, and can include them on your package labels. This is only applicable to test results added after this release.

The new data keys for these fields are "package.lab_results_reference.x.lab_results.custom_field_y" where x is the lab result index and y is the custom field number (1 to 5).
